We are excited to welcome Jordan Breckon, WNYICC's newest employee, to the WNYICC Team!

Jordan started with Western New York Integrated Care Collaborative in mid-July and her impact on the organization has been felt immediately.

As our Operations Manager, Jordan is responsible for ensuring the smooth, efficent, customer-driven operations at WNYICC, both internally and externally.  

The timing of her hire could not have been better.  WNYICC has launched our first program this month with referrals coming in and being sent downstream to our network members.  Jordan has quickly become the referrals and portal expert.  She will be involved with training, quality assurance, and general day-to-day functioning of WNYICC.

Not only is Jordan extremely talented -  network members, contract partners and vendors will soon learn that she is easy to work with, extremely helpful and knowledgeable about all things WNYICC!

Jordan's previous positions at Erie County Department of Senior Services paved the way for her work at WNYICC.  There she held the positions of Health and Wellness Coordinator followed by RSVP (Retired Senior Volunteer Program) Coordinator.   She is a trained Peer Leader in the Chronic Disease and Diabetes Self-Management Programs, a Master Coach in the Matter of Balance Program, as well as a Master Coach in the Aging Mastery Program.
