  • Programs are delivered by a regional network of trusted partners with established expertise in health promotion services.
  • Evidence-based programs that have demonstrated improved outcomes.

  • Currently covering 8 counties of Western New York (Erie, Niagara, Chautauqua, Orleans, Wyoming, Genesee, Cattaraugus, and Allegany). 
  • Menus are prepared using fresh ingredients.
  • Menus are approved by a Registered Dietitian and meet the Recommended Dietary Allowance (RDA) for nutrients.
  • Must have specific Independent Health Medicare Advantage Plans 
  • Integrated Care's Community Health Coaching program (CHC) is designed to empower individuals with chronic conditions to address social factors occurring in their home in order to better manage their care and improve their quality of life, through the assistance of a Health Coach.
  • Must have Independent Health Medicare Advantage plan
  • This program is a yearlong intervention proven to reduce the risk of developing diabetes provided over a series of 22 group workshops
  • WNYICC's National Diabetes Prevention Program (NDPP) is fully recognized by the Centers for Disease Control (CDC)
  • We accept many health plans - submit your registration to check eligibility.
  • Certified Healthy IDEAS Health Coaches work with clients on an individual basis ver 3-12 months.

  • Evidence-based program for indiviudals with symptoms of social isolation or depression that integrates depression awareness, education, referrals, linkages, and behavioral activation.

  • Must have Independent Health Medicare Advantage plan

  • Integrated Care's Falls Prevention Program is designed to empower individuals with a history or fear of falling to address social factors occurring in their home in order to better manage their care and improve their quality of life, through the assistance of a Health Coach over 6 months.
  • Participants receive one-on-one coaching (including falls risk assessment and care planning) PLUS enrollment into an evidence-based Falls Prevention group workshop.
  • Must have Independent Health Medicare Advantage plan
  • Integrated Care's Caregiver Support Program is designed to empower individuals and their caregivers to address health care social risk factors and caregiver needs in order to better manage their care and improve their quality of life, through the assistance of a Health Coach over 6 months.
  • Participants receive one-on-one coaching (including caregiver assessment and care plan) PLUS enrollment into an evidence-based Caregiver group workshop.
  • Must have Independent Health Medicare Advantage plan

 

  • Integrated Care's Accredited Diabetes Self-Management Education and Support (DSMES) Program.
  • Program combines Individual Nutrition Counseling with a Registered Dietitian PLUS an evidence-based facilitated Group Diabetes Self-Management Program delivered by our trusted service network members.
  • We accept Independent Health (Commercial, Medicare) and Univera (Commercial, Medicare)
  • Evidence-based group workshops developed by Stanford University
  • Led by certified Peer Leaders 
  • Participants report pain reduction, improved health, increased physical activity, decreased social isolation, improved relationship with healthcare providers
  • Any adult over 18 with chronic (long-lasting) condition or pain is welcome!
