WNYICC Diabetes Prevention Program (DPP):

Our trusted service network of community partners deliver our evidence-based DPP program for people with pre-diabetes and at high-risk for developing Type II Diabetes.

 

Are you or someone you love at risk for developing diabetes? Our Diabetes Prevention Program can help lower your risk of developing diabetes by at least 58%!

 

WNYICC Diabetes Prevention Program Facts:

  • Full Recognition from the CDC
  •  Led by a trained Lifestyle Coach utilizing the Prevent T2 curriculum
  • Group sessions over 12 months: 1 hour per week for 12 weeks, then taper to monthly sessions.
  • The program is delivered in community settings, in groups of 10-15 people.
  • Personal lifestyle goals are set by each participant.
  • Convenient: Delivered in-person at community locations or virtually
  • The sessions cover healthy eating, physical activity, and lifestyle changes.

Goals of Program:

  •  Prevention or delay of a diabetes diagnosis

  • 5-7% weight loss and maintenance
  • Gradual increase in physical activity to 150 minutes per week

Eligibility:

  • Participants must have Pre-Diabetes diagnosis as evidenced by blood glucose or A1c labwork
  • Medicaid participants must have sign referral from primary care.
  • Participant's BMI must be > 25
  • Currently accepting following health insurance plans:

    • Univera Medicare Advantage, Commerical, and Medicaid Managed Care
    • Highmark / BlueCross BlueShield of WNY Medicare Advantage

    • Fidelis Medicaid Managed Care
    • Original Medicare
    • NYS Medicaid
