Free Peer Leader Training for Living Healthy Self-Management Programs in June

Graduates of the training will be certified to facilitate the Living Healthy Chronic Disease and Diabetes Self-Management Programs offered in partnership between WNYICC and Erie County Senior Services and Niagara Co. Sept. of Health.

WNYICC accepted into NCOA’s Medicare Reimbursement Learning Collaborative

WNY Integrated Care has been accepted into the National Council on Aging’s 2018-2019 Center for Healthy Aging Medicare Reimbursement Learning Collaborative (MRLC).

Living Healthy Diabetes Self-Management Program

This 6-week workshop helps participants learn or improve problem-solving, goal setting and decision-making skills, enabling them to confront the challenges of living with diabetes.  Caregivers are also welcome.
